Welcome to the 2017 Harvest Festival® Fall Tour. Food exhibitors are a very important part of our festival and we appreciate your business. Harvest Festival® is required by law, to provide the following information to the Health Departments in each show city. Harvest Festival® collects all forms and will send them in to the various departments with payment on your behalf. REGARDLESS OF WHAT THE APPLICATION DIRECTS YOU TO DO, PLEASE DO NOT SEND DIRECTLY TO THE HEALTH DEPARTMENT!
Each Food Vendor MUST provide the following to the Harvest Festival® office by:

AUGUST 11, 2017 for ALL OTHER SHOWS

THERE WILL BE NO EXCEPTIONS TO THE RULES. ANY EXHIBITOR THAT DOES NOT COMPLY WITH THESE REQUIREMENTS WILL BE SUBJECT TO CANCELLATION WITHOUT A REFUND OF BOOTH FEES.
Make ALL checks payable to Harvest Festival®. For Credit Card payments, please fill in the credit card authorization form and send in with all other required paperwork to our office at the address below.
